Appointments and Cancellations

By booking an appointment through our website or Jane App booking system, you agree to the following:

  • Appointments are confirmed upon booking and a confirmation will be sent to your email.
  • We require at least 24 hours notice for cancellations or rescheduling.
  • Late cancellations (less than 24 hours) or no-shows may be subject to a cancellation fee.
  • We reserve the right to modify our cancellation policy with reasonable notice.

Payment and Insurance

  • Payment is due at the time of service unless other arrangements have been made.
  • We accept cash, credit cards, and HSA/FSA cards.
  • We are in-network with Blue Cross Blue Shield and United Healthcare at our Schaumburg location.
  • We provide out-of-network superbills for Cigna, Aetna, and other insurance carriers upon request.
  • Insurance coverage varies by plan. It is your responsibility to verify your benefits before your appointment.
  • We are not responsible for claims denied by your insurance provider.
