Appointments and Cancellations
By booking an appointment through our website or Jane App booking system, you agree to the following:
- Appointments are confirmed upon booking and a confirmation will be sent to your email.
- We require at least 24 hours notice for cancellations or rescheduling.
- Late cancellations (less than 24 hours) or no-shows may be subject to a cancellation fee.

Payment and Insurance

- We are in-network with Blue Cross Blue Shield and United Healthcare at our Schaumburg location.
- We provide out-of-network superbills for Cigna, Aetna, and other insurance carriers upon request.

- We are not responsible for claims denied by your insurance provider.
